Create a login / Initial application form:
You must create a Login ID and Password that will give you access to the Online Application system. You can begin the process by selecting the First Time User link or the First Time User button below. Follow the instructions provided to create a Login ID, password and security questions. Once these are successfully created you can log on with the new Login ID and Password. Then you may select a License Type and begin filling out the appropriate application online. You may save an incomplete application and return to it at any time using the login information.
Answer Questions:
Access the mandatory questions page. Answer all questions and select "Save" to see a confirmation of all your answers. If the answers are correct select "Continue" at the bottom to finish the remaining sections of the applicatioin. If you need to change any answers, click on the "Questions" box and make the necessary changes. Answering "YES" to any of these questions will not necessarily prevent a license from being issued by the state.
Make a Payment:
Once the application is 100% complete and the mandatory questions are answered, you will have to submit the application fee specific to your license. Select a payment type and select "Continue". If you select "Credit Card" or "Electronic Funds Transfer", enter all the required fields and "Continue". Verify your payment information and select "Process Payment". The system will process your information and produce a receipt that you can print for your records.
NOTE: Application fees are non-refundable.
Send Supporting Documentation:
Once you have completed the initial application form for your license, you will be required to submit additional materials.
Once all four of the above steps are complete and Committee/OSBMLS have received all of your documentation, including documentation from third parties, your application will be submitted to the appropriate committee/board for final approval. You will be able to monitor the progress of your application and view any deficiencies via the website.
