Your complete 9-digit social security number (SSN) is not required. OSBI requires a minimum of the last 4 digits of your SSN to verify identity. Failure to include your complete 9-digit SSN may delay processing of your application.

Do not include suffix information (i.e. JR, SR, II, etc...) in the last name field.

Previous applicants: enter your last name as it appears on your SDA license card or on file with the OSBI (minus any suffix information).
If your last name has changed since your previous application,
you will be able to make that change on a later screen.
